The State of Illinois requires all kindergarteners, 6th and 9th graders to have a school physical and proof of immunizations. New Pre-K students are also required to have a physical.
Kindergarteners need a dental and vision exam.
2nd graders, 6th and 9th graders need a dental exam.
Eye exams are due by October 15, 2026 and dental exams are due May 15, 2027.
If you are a family that will be new to the district from out of state your student will also be required to have an Illinois physical and vaccine record on file.
PLEASE REMEMBER: Sports physicals are NOT school physicals. All school physicals must be on the form below. However, your doctor may put a sports physical on a school physical form!

📚 Online Registration Opens Next Week! 📚
It’s almost time to get ready for the new school year! Online registration for the 2026–2027 school year opens Monday, July 20 through TeacherEase.
If you prefer to register in person or need assistance with the online process, our school office will be open the first week of August from 9:00 a.m. to 3:00 p.m. Staff will be happy to help, and you may also bring any required paperwork with you at that time.

As you're checking off your back-to-school to-do list, don't forget about your student’s health requirements!
Illinois School Health Requirements
Preschool – Physical exam & required immunizations
Kindergarten – Physical exam, required immunizations, dental exam, and vision exam
2nd Grade – Dental exam
6th Grade – Physical exam, required immunizations, and dental exam
9th Grade – Physical exam & required immunizations
12th Grade – Proof of the second dose of the Meningococcal (MCV4) vaccine
Student Athletes – A current physical is required to participate in school sports.
If your child has:
Daily or emergency medications
Food allergies
Asthma
Diabetes
Seizures or another medical condition requiring care at school
Please contact the school nurse (shelbi.smith@cusd17.com) before the first day of school so we can make sure everything is in place for a safe and successful year.
Appointments fill up quickly this time of year! If your child still needs a physical, immunizations, or dental/vision exam, we encourage you to schedule those appointments as soon as possible.

The Senior Beta Club attended National Beta Convention this week at Nashville, TN. Mrs. Reiman is ecstatic to share the following awards we are bringing home from convention.
Matthew Marks - 7th place Division I B&W Photography
Alyssa Fabian - 2nd Place Division II Poetry
Autumn Ebersohl - 1st Place Division II Woodworking.
Mrs. Reiman is proud of all the students who competed at Nationals. They truly represented Cobden High School well.
