If your child will be 5 years old by September 1, 2021 and you live in the Cobden School District, please call 893-2311 to make a kindergarten screening appointment.
Please bring your child and ALL of the information listed below to your appointment.
-Child's Birth Certificate (must be a certified birth certificate, not the hospital copy)
-Immunization Record
-Proof of Income (W2's, at least 3 pay stubs, tax return)
-Proof of Residency in Cobden School District (utility bill, tax bill, official document, etc. Must have physical address on it)
