Wednesday, August 13, 2008, 12:00 pm-6:00 pm
PHYSICALS ARE REQUIRED FOR KINDERGARTEN, 6TH & 9TH GRADES
(due to the change, 5th graders who had a physical last year and were in compliance
are not required to have the physical again this year.)
ALL STUDENTS WHO INTEND TO TRY-OUT AND PARTICIPATE IN ATHLETICS
ARE REQUIRED TO HAVE A SPORTS PHYSICAL
(a physical is required for try-outs!)
Please remember these things:
1. A sports physical WILL NOT meet the requirement for a regular physical;
please be sure to have your physician complete BOTH forms if you are required to have both!
2. Parents, please remember to compete the patient history section of the physical forms;
Cobden District #17 cannot accept incomplete physical forms!
DENTAL EXAMS ARE REQUIRED FOR KINDERGARTEN, 2ND, & 6TH GRADES.
